Transfusion-dependent Anemia Treatment Market Size and Trends Analysis

The global transfusion-dependent anemia treatment market size is likely to be valued at US$4.6 billion in 2026 and is estimated to reach US$9.8 billion by 2033, growing at a CAGR of 11.4% during the forecast period 2026 to 2033, driven by new drug approvals, gene therapies, and increasing use in myelodysplastic syndromes and thalassemia. Rising transfusion costs also push payers to back therapies that cut transfusions.

Key Industry Highlights

  • Leading Therapy Type: Erythroid maturation agents, about 52.8% share in 2026, because luspatercept holds approvals in both thalassemia and myelodysplastic syndromes.
  • Dominant Indication: Myelodysplastic syndromes, around 54.2% share in 2026, as many older patients become transfusion dependent.
  • Leading Region: North America, with about 46.3% share in 2026, owing to early approvals and high drug uptake.
  • Fast-growing Region: Asia Pacific, backed by a high thalassemia burden and growing access to new therapies.
  • Latest Approval: In December 2025, Agios Pharmaceuticals announced that the U.S. Food and Drug Administration (FDA) had approved AQVESME (mitapivat) for treating anemia in adults with alpha- or beta-thalassemia, including transfusion-dependent patients. The oral pyruvate kinase activator was supported by the Phase 3 ENERGIZE and ENERGIZE-T trials, which demonstrated improvements in anemia-related outcomes and reductions in transfusion burden. The approval introduced an oral treatment option for eligible adults with transfusion-dependent thalassemia, expanding the range of therapeutic approaches beyond injectable medicines and transfusions.

DRO Analysis

Driver- Surging Uptake of New Disease-modifying Drugs to Augment Demand

Transfusion-dependent anemia needs regular red blood cell transfusions. It occurs in beta thalassemia and in lower-risk myelodysplastic syndromes (MDS). Transfusions bring iron overload and a heavy care burden. New drugs aim to cut or end transfusions. Bristol Myers Squibb reported Reblozyl sales of US$735 million in the second quarter of 2026, up 29%. Reblozyl also leads the company's growth portfolio, which rose 14% to US$7.6 billion in the quarter. Its international sales rose 24% to US$142 million.

Reblozyl (luspatercept) is the market anchor. The FDA approved it on November 8, 2019, for beta thalassemia. In the BELIEVE trial, 21.4% of patients achieved at least a 33% cut in transfusion burden, against 4.5% on placebo. U.S. sales reached US$593 million in the quarter, up 31%. The dose is 1 mg/kg given under the skin once every three weeks. The trial enrolled 336 adults, with 224 on luspatercept and 112 on placebo.

Expanded Labels and New Mechanisms to Strengthen Long-term Market Growth

Labels keep widening. On August 28, 2023, the FDA approved luspatercept as first-line treatment for anemia in lower-risk MDS. The COMMANDS trial enrolled 301 ESA-naive patients. Transfusion independence of at least 12 weeks reached 58.5% with luspatercept and 31.2% with epoetin alfa. Treatment-related adverse events occurred in 30.3% of luspatercept patients and 17.6% of epoetin patients. The label is for adults who have not received ESAs.

New mechanisms add choice. The FDA approved Rytelo (imetelstat) on June 6, 2024, for lower-risk MDS with transfusion-dependent anemia. In the IMerge trial, 40% of patients achieved eight-week transfusion independence, against 15% on placebo. At 24 weeks, the rates were 28% and 3.3%. The IMerge trial enrolled 178 adults at 77 sites in 17 countries. It served patients who needed four or more red blood cell units over eight weeks and had failed ESAs.

Restraint- High Treatment Cost and Safety Monitoring May Hinder Market Growth

Costs are high. NICE set the list price of Casgevy at £1,651,000 (approx. US$2.2 million) per course of treatment. Vertex's U.S. price for the sickle cell indication was US$2.2 million. Payers review such prices closely. Gene therapy also needs stem cell transplant expertise, and only a few centers can offer it. Aqvesme's boxed warning follows five reports of reactions suggestive of liver injury. Two patients were hospitalized. All reactions began within six months and improved after the drug was stopped.

Safety rules add friction. Aqvesme (mitapivat) carries a boxed warning for liver injury. Liver tests are required every four weeks for the first 24 weeks. Access is restricted through a risk program. Rytelo can cause low platelet and neutrophil counts. These risks require careful monitoring. The most common reactions to Aqvesme were headache and insomnia. Rytelo's label also warns of infusion reactions and harm to unborn babies.

Opportunity- Gene Therapy and Oral Drugs to Support New Revenue

Gene therapy offers a one-time option. Casgevy is a CRISPR-based therapy approved by the FDA in January 2024 for transfusion-dependent beta thalassemia in patients aged 12 and older. About 1,000 U.S. patients are eligible. Nine authorized treatment centers offered the therapy at launch. Casgevy uses CRISPR gene editing to modify a patient's own blood stem cells. It came more than two months before its target action date. It was earlier approved for sickle cell disease in December 2023.

Oral therapy adds reach. Agios announced FDA approval of Aqvesme on December 23, 2025. It is an oral pyruvate kinase activator for adults with alpha- or beta-thalassemia. The ENERGIZE-T trial enrolled 258 transfusion-dependent patients. About 6,000 adults in the U.S. are diagnosed with thalassemia. ENERGIZE-T asked whether patients could cut transfused red cell units by at least 50% through Week 48. The ENERGIZE trial enrolled 194 non-transfusion-dependent patients. Together, 452 patients took part.

Label Expansion and Emerging Markets to Create New Demand Pools

Pipeline expansion is under way. The FDA accepted a Reblozyl application for use with a JAK inhibitor in myelofibrosis-associated anemia. The INDEPENDENCE study supports the filing. The FDA set a target action date of March 11, 2027. A new label would open another transfusion-dependent group.

Emerging markets hold a large unmet need. India has about 150,000 thalassemia patients and about 12,000 new cases each year, according to a government-linked summary. Carrier rates are 3% to 4%. Rising screening and public programs should lift diagnosis and treatment. The Thalassemia Bal Sewa Yojana is funded by Coal India's corporate social responsibility budget. Phase II, since 2021, also covers aplastic anemia.

Category-wise Analysis

Therapy Type Insights

Erythroid maturation agents are predicted to lead with a share of about 52.8% in 2026, because luspatercept holds approvals in both thalassemia and MDS. Reblozyl is given subcutaneously at 1 mg/kg every three weeks in thalassemia. Bristol Myers Squibb counts it as a key growth asset. Rytelo is a telomerase inhibitor for patients who have lost response to ESAs.

Gene therapies are estimated to be the fastest-growing therapy type in the forecast period, as one-time treatments gain reimbursement. NICE recommended Casgevy for up to 460 people in England. This is our analyst view. Public sales splits by therapy type are limited. The list price of £1,651,000 is a barrier. Funding through the Innovative Medicines Fund lets patients get treatment while more evidence is collected.

Indication Insights

Myelodysplastic syndromes are anticipated to dominate with a share of around 54.2% in 2026, because many older patients become transfusion dependent. In the IMerge trial, the median age was 72. About 80% of patients were 65 or older. Two approved drugs now target this group. COMMANDS showed that luspatercept beat epoetin alfa in first-line use. Rytelo serves patients who have lost response to ESAs.

Beta thalassemia is expected to be the fastest-growing indication in the forecast period, as gene therapy and oral drugs add new options. Luspatercept, mitapivat, and Casgevy all hold approvals. This is our analyst view. Patients are younger, so treatment lasts longer. Aqvesme's ENERGIZE-T trial enrolled 258 patients in this group.

Key Industry Developments

  • In July 2026, Vertex Pharmaceuticals announced that the U.S. FDA had expanded approval of CASGEVY (exagamglogene autotemcel), a CRISPR-based gene-editing therapy, to include patients as young as two years old with transfusion-dependent beta-thalassemia. Previously, the therapy was approved for patients aged 12 years and older. The expanded indication was supported by clinical data in younger children, including transfusion independence lasting at least 12 consecutive months in eight of nine evaluable patients with transfusion-dependent beta-thalassemia. The decision broadened access to a one-time gene therapy intended to address the underlying disease mechanism.
  • In June 2026, Takeda initiated a Phase 3 clinical trial evaluating elritercept against epoetin alfa in adults with lower-risk myelodysplastic syndromes (MDS) who require red blood cell transfusions. The trial plans to enroll approximately 304 participants and will assess whether elritercept can increase transfusion independence while improving hemoglobin levels. The study represents a further effort to develop treatments that reduce transfusion requirements in patients with MDS-associated anemia.
  • In February 2026, Bristol Myers Squibb announced positive topline results from a registrational Phase 2 study evaluating luspatercept in adults with alpha-thalassemia. Both the transfusion-dependent and non-transfusion-dependent patient groups met their respective primary endpoints. In the transfusion-dependent cohort, luspatercept produced a statistically significant reduction in red blood cell transfusion burden compared with placebo. The results support further development of the therapy for alpha-thalassemia, a condition with limited treatment options, and the company planned to discuss the findings with China’s Center for Drug Evaluation.
